this isn't making a whole lot of sense to me...
are you creating items with our website (ie: not CCpublisher or from ourmedia)?
if so, the fact that your smartFTP client is automatically cd-ing into a subdir is likely something specific to your client -- we set up the FTP urls to connect to with the correct subdir information for each new item -- so our site should not be steering you into an older item's subdir.
and i'm not sure i can see how "_" vs "__" is working...

I create the identifiers through the "Item Creation Page" http://www.archive.org/create/ under the section "You may use FTP to directly add items to the Archive:"
It just happened again so here is the information. This is the idenfier I used:
THS_73_HappinessAndEverythingHappeningForTheBest_43kb_wmv
This is the Page it took me to:
http://www.archive.org/create.php?identifier=THS_73_HappinessAndEverythingHappeningForTheBest_43kb_wmv
This is the FTP host name that the identifier was assigned:
ia300238.us.archive.org
This host name (ia300238), however, is the same one I was assigned for a file that I uploaded about ten minutes before that had the identifier: THS_77_HappyHouseARealityTelevisionIdea_43kb_wmv.
My FTP program (SmartFTP) took me to that folder.
This run was better than usual. I uploaded 6 files before the problem developed (usually it develops after 3 or 4). It may help to know that the wrong address is usually (perhaps always) one that I had used to upload a file several minutes earlier.
Again, I'm always able to resolve this problem successfully by slightly changing the identifier (like adding a second underscore).

ok, i'm "masquerading" as you...
if you go to
http://www.archive.org/create.php?identifier=THS_73_HappinessAndEverythingHappeningForTheBest_43kb_wmv
you'll notice that it says:
Change directory to subdir "THS_73_HappinessAndEverythingHappeningForTheBest_43kb_wmv" and upload, replace, or delete files.
so i think your smartFTP client is getting "overeager"
and caching some prior dir and thus trying to change dir to a prior subdir.
i'm looking on the FTP server, too, and i see the subdir
THS_73_HappinessAndEverythingHappeningForTheBest_43kb_wmv
can you look for it now?
possibly "cd .." and then "cd " after smartftp
connects.
